Disclaimers & Informed Consent
Welcome, and thank you for considering therapy with me. It is important that you understand your rights and responsibilities as a client, as well as the nature and limitations of counseling services. Please read this information carefully.
Nature of Therapy
Therapy is a process of growth and change. While it can bring about increased self-awareness, improved relationships, and greater coping skills, it may also involve discussing difficult or painful experiences. Progress varies from person to person, and no outcome can be guaranteed.
Confidentiality
Your privacy is very important to me. Information shared in sessions is confidential, with the following legal exceptions:
• Suspected child, elder, or dependent adult abuse.
• Imminent risk of serious harm to yourself or others.
• Court orders or other legal requirements.
In these cases, I am ethically and legally obligated to take appropriate steps, which may include contacting protective services, emergency responders, or legal authorities.
Client Rights
As a client, you have the right to:
• Ask questions about your therapy and my methods.
• Refuse or discontinue treatment at any time.
• Request referrals to other providers if needed.
• Review your records, as permitted by law.
Fees & Policies
• Session length and fees will be outlined at the time of intake.
• Payment is due at each session unless other arrangements are made.
• Missed or late-canceled sessions (without 48-hour notice) will result in a $75 cancellation fee.
If you plan to use insurance, please confirm your coverage in advance.
Telehealth Services
If services are provided via telehealth, please note:
• You must be physically located in the state(s) where I am licensed at the time of your session.
• Technology has risks, including possible interruptions, unauthorized access, or technical failures.
• It is your responsibility to ensure you are in a private and safe environment for sessions.
Limits of Services
Therapy is not a crisis service. If you are experiencing a life-threatening emergency, please call 911 immediately or go to your nearest emergency department.
For mental health crises, you may also call or text 988 (Suicide & Crisis Lifeline in the U.S.) or contact your local crisis line.
